ELEPHANT IMMUNISATIONS



The Pentavalent vaccine gives protection against 5 severe and contagious diseases. A form of this vaccine is given all over the world, yet 2.2 million children in Nigeria have not had a single dose, let alone the full 3 doses. Donors have offered to reward state governments in Nigeria for every percentage point increase in Penta 3 uptake.


GOAL
Increase uptake of Penta 3 vaccinations, and provide clear evidence of increase to governments and donors.

DISCOVERY RESEARCH
I began by looking into vaccine hesitancy in Nigeria and behaviour change approaches specific to driving vaccine uptake. This was a mixture of desk research into existing studies and vaccination programmes, and discovery research through our in-country teams.

This led to loads of great learnings, including the 3 Cs of Vaccine Hesitancy, which I used as the basis for an ideation workshop.

COLLABORATION & FACILITATION
To gather as many ideas as quickly as possible, I organised a workshop for members of the product and engineering team, and some of our in-country team. Everyone was split into 3 groups, each focused on tackling one of the Cs of vaccine hesitancy.

We came out with so many amazing ideas to increase Penta 3 uptake. Two main buckets that emerged were:
1. Clinical decision support - helping clinicians to understand a child's vaccine status and guiding them on correct steps
2. Patient communication - helping guardians to understand when and why they should bring their child for a vaccination

UI & UX DESIGN
Beginning with the clinical decision support, I set about making the process for documenting vaccines as simple and quick as possible. Our users are functioning in a busy, noisy environment, so giving them a simple process was important for usage. We have a mantra of Taps, Not Types. Every keyboard interaction is time consuming and cumbersome. To aid clinicians, I added quick actions to point them in the right direction, as well as exposing a clear summary of the patient's vaccine status up front.

There were many complex cases during this process. One particularly challenging aspect was making sure this system was suitable for the reality of our market. Full doses, particularly for Penta, aren't always available. So the system needed to be flexible enough to handle these instances.

USABILITY RESEARCH
During the design phase we ran quick usability research with users in facilities. This gave some immediate feedback on content and vaccine naming conventions, as well as more optimisations for future iterations.
High level usability findings
OUTCOMES
The end user journey contained a number of features to improve the efficiency and quality of clinical care. There is a clear vaccination status shown upfront when the patient record is loaded, with buttons to add due vaccinations quickly. There are clinical nudges built into the documentation process to catch any incorrect vaccinations or missed documentation.

We saw positive metrics for both documentation efficiency and in uptake in Penta 3, and received a lot of positive user feedback about the improved ease of use.
